Mike Berkley, thank you for reporting this and helping make Ubuntu better. 
Please execute the following command, as it will automatically gather debugging 
information, in a terminal:
apport-collect 218903

As well, could you please capture the oops following
https://wiki.ubuntu.com/KernelTeam/KernelTeamBugPolicies#Capturing_OOPs
? In addition, if you could also please test the latest upstream kernel
available that would be great. It will allow additional upstream
developers to examine the issue. Refer to
https://wiki.ubuntu.com/KernelMainlineBuilds . Once you've tested the
upstream kernel, please remove the 'needs-upstream-testing' tag. This
can be done by clicking on the yellow pencil icon next to the tag
located at the bottom of the bug description and deleting the 'needs-
upstream-testing' text.

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not
boot, please add the tag: 'kernel-unable-to-test-upstream'.

The machine on which this problem occurred has been retired, so the
problem is no longer reproducable.

Hello,

I am getting exactly the same problem.

I am using an IBM System x3400 with the Adaptec 29320LP Ultra320 SCSI adapter 
card, and HP Ultrium 320 Tape Drive
I am using Ubuntu Server edition (8.04) (64bit) 

All equipment is new - and tape drive works with windows software

The Ubuntu Kernel Team is planning to move to the 2.6.27 kernel for the
upcoming Intrepid Ibex 8.10 release.  As a result, the kernel team would
appreciate it if you could please test this newer 2.6.27 Ubuntu kernel.
There are one of two ways you should be able to test:

1)  If you are comfortable installing packages on your own, the linux-
image-2.6.27-* package is currently available for you to install and
test.

--or--

2)  The upcoming Alpha5 for Intrepid Ibex 8.10 will contain this newer
2.6.27 Ubuntu kernel.  Alpha5 is set to be released Thursday Sept 4.
Please watch http://www.ubuntu.com/testing for Alpha5 to be announced.
You should then be able to test via a LiveCD.

Please let us know immediately if this newer 2.6.27 kernel resolves the
bug reported here or if the issue remains.  More importantly, please
open a new bug report for each new bug/regression introduced by the
2.6.27 kernel and tag the bug report with 'linux-2.6.27'.  Also, please
specifically note if the issue does or does not appear in the 2.6.26
kernel.  Thanks again, we really appreicate your help and feedback.

Here is the dmesg output, when the Adaptec 29320LPE is running in U320
mode.  When it is restricted to U160 mode, dmesg output is normal,
without SCSI errors.

So far, I have been unable to get the newer driver to compile.  As soon
as I fix one undefined symbol, and another one appears.  It looks like
the aic7xxx directory shipped with Hardy has been modified from the
original Adaptec driver, with a large number of coding repairs.

I was able to get the card to work by reducing the bus speed from U320
to U160.  I may have to live with this, because I need to get the server
into service.

I will revert back to U320, and post dmesg output as soon as I can.
