In Ubuntu karmic, there is still no menu item. As upstream doesn't provide one, I think the package maintainer should add an own and remove the Debian-specific menu entry which apparently is ignored. Thanks for reporting this bug and any supporting documentation. When mapivi is installed, there is no .desktop file created, thus there is no menu item created. Since this bug has enough information provided for a developer to begin work, I'm going to mark it as confirmed and let them handle it from here.
