This happens to me as well.  I use the dvorak and US layout.  Also, when
doing '+ command', it only does it in the system default keyboard
layout.  For example, in the US layout, the letter C is actually
dvorak's letter J.  So, on my machine where US is the default and I need
to kill a command in the terminal (normally +c), I actually have
to type +j. The same thing occurs when dvorak is the default
layout and I'm using the US layout.

I can confirm that this bug appears in Ubuntu 10.04, Lucid Lynx. Oddly
enough, I did not encounter it in 9.10. Anyway, for professional
reasons, I generally switch between three different keyboard layouts:
USA, USA International (with dead keys), and Latin American. I have
configured GNOME to change the keyboard layout when I press Shift + Caps
Lock. However, since upgrading to Lucid, the keyboard configuration will
randomly switch without me entering the key sequence. Thus far, I have
been unable to isolate a single common key sequence or action that
triggers this change reliably, although I have found that using the
mouse to select an item from a GNOME panel frequently triggers it. I
have examined the hotkey settings, and found nothing that would explain
why the layout keeps changing. As per the previous request, I am
attaching a file containing my hardware profile and X log to this
message.

Right now it doesn't change keyboard to US international (dead keys) it
changes to whatever I have defined in xorg.conf

My conclusion is that settings in Gnome are being ignored in favor of
the settings in xorg.conf whenever I plug in an USB keyboard. A one way
to reset it, is by going to keyboard preferences, then adding or
removing any keyboard.

I guess the core issue is that Ubuntu doesn't use the keyboard settings
from xorg.conf (since it has its own program to set up the keyboard),
but plugging the usb keyboard and mouse resets keyboard settings to ones
in the config file.

It seems that usually happens when I plug in an USB keyboard.
And the keymapping are coming from xorg.conf.
Though, sometimes the keyboard indicator shows that I have 3 different keyboard 
layouts, one is USA and 2 other are some weird characters (like a tiny "z" etc).
