This one affects me as well.  Muting mutes the master, and headphone and
PCM.  Unmuting only unmutes Master.

I see this on Ubuntu 12.10.

Start with sound working, then do:

$ amixer -Dhw:0 set 'Master' mute
Simple mixer control 'Master',0

  Limits: Playback 0 - 64
  Mono: Playback 53 [83%] [-11.00dB] [off]

Sound is muted, as expected.  The same thing happens if you mute *any*
channel (not just master) in alsamixer.

$ amixer -Dhw:0 set 'Master' unmute
Simple mixer control 'Master',0
  Capabilities: pvolume pvolume-joined pswitch pswitch-joined penum
  Playback channels: Mono
  Limits: Playback 0 - 64
  Mono: Playback 53 [83%] [-11.00dB] [on]

Sound is still muted, both in reality and as shown by xfce4-volumed.  If
I open alsamixer, it shows master unmuted but everything else muted (if
I had used alsamixer to do the muting, it would not recognize this until
alsamixer is restarted).  If I unmute *all* output channels (even ones I
don't have anything attached to), the actual sound finally resumes (and
xfce4-volumed mute indication changes) when I unmute the last one.

"amixer -D pulse set Master 1+ unmute" also works to unmute all the
channels.

It seems that pulseaudio can't handle anything but a single system-wide
mute switch, which is bad enough, and simulates multi-channel mute
poorly (if muting one channel automatically mutes all, why does unmuting
one not unmute all?), leaving confused mixing apps and confused users
who can't see an easy way to turn their sound back on.

I am using: 
   UBUNTU 10.04 LTS
   GNOME 2.30.2  6/25/2010

This works:  amixer  set Master mute 
This does NOT work: amixer  set Master unmute 

When I execute
  >>> amixer  set Master mute 
the GUI (Sound Preferences) shows   OUTPUT VOLUME MUTE and there is no sound
whether or not sound output was previously muted

HOWEVER, when I  execute
 >>> amixer set master unmute
at a time when sound output is muted
"amixer" seems to think it has toggled "mute" but the GUI  (Sound Preferences)  
OUTPUT VOLUME  STAYS MUTED

BTW, I want a means to set sound output to MUTE upon LOGIN.

I've seen that toggling mute with ALSA (amixer) can leave PulseAudio in
a muted state and the only way to unmute it is to run 'pactl set-sink-
mute $CARDS 1' where $CARDS is the index of your audio sink (you can try
to replace it with 0 or 1). I've modified a simple script that you can
bind to the XF86AudioMute key to toggle mute. It will toggle mute for
all the sinks present which usually isn't an issue. The directions for
the key binding in the script are for XFCE (I use Xubuntu) so yours may
be in another location. Let me know if this helps.
