I can confirm the bug on 64-bit 12.10. I am also seeing this behavior with a logitech HD Webcam C310. I am running ubuntu 12.10 and am receiving the same initialization failed error. I've had the same issue with Ubuntu 11.10 and also in 12.04 with pulseaudio version 1.1-0ubuntu15, both 64-bit. It occurs about 90% of every suspend/resume when usb webcam (Logitech C270) is plugged in.
