I also have this problem. When I log in, most of the time the borders are there and it's fine. The problem often occurs when dragging a window from fullscreen on one monitor to fullscren on another. I click up in the upper unity task bar, where the menu goes when a window is fullscreen. I drag it down, and then up into the task bar on my other monitor. Once it goes fullscreen, I lose my window borders and menus for all my other open programs. I encountered this problem whenever whenever open a Java swing application, In my case, I opened Jira client and I couldn't see the windows decorator. It would appear that configuring different plugins through CCSM unsettles a lot of other plugins, including 'Window Decoration'.
