The use case for this is since there is no tapAndHold I am attempting to
bake my own - my idea for this was to:

timer = setTimeout(handleTapAndHold, 800) onGestureStart
clearTimeout(timer) onGestureEnd

I don't think onGestureOccurred fulfills this requirement?

You're correct:  for that use case the TOUCH gesture primitive (which
has start/end semantics) needs to be made available through the QML
bindings.

I'm thinking this refers to bad documentation. The documentation doesn't
really distinguish between continuous and instantaneous gestures.
