I've been monitoring, and as as near as I can ascertain its a combination of flash adverts and tabs. When there is flash content and I deliberately give it focus, ctrl-W stops working. While thats annoying as heck, its not the primary issue here. So what I think is happening is that when working with many tabs, somehow a flash advert gets focus on a tab, and ctrl-W then stops working until you click on the page outside the advert. I have experienced this. If I have loads of tabs open, It has to be more than will fit on the tab bar, so the arrow appears at the end on the right. THen when I start hitting Ctrl + W to start closing them, they will close successfully, until there are enough to just fill the tab bar. I note this behavior on some pages that have flash content. It's as if the flash applet has the focus and the parent tab (window) doesn't see the CTRL+W keypress. In addition CTRL+W doesn't work if the page has not yet loaded. For example I have a bookmark folder that has a number of sites listed, I open all of them in tabs (only about 6 pages) then read them and close them as I go with CTRL+W. If i get to a tab that is having a problem due to the server not responding, CTRL+W does nothing. I have to press ESC to stop that tab from attempting to load, then press CTRL+W to close it. Apparently there is some debate over the desirability, but it is intentional. Are you sure firefox was the window with the focus? to use ctrl+w it needs to be the focused window. sounds like you had to click the emtpy space becasue the firefox window wasnt the focused window. Please make sure thats what it is. I usually encounter this when I have a range of tabs open and want to close them (but make sure to check if I really don't need one of them still). My native window manager is ion3 and browser version is Debian-2.0.0.1+dfsg-2.
