I'm using 12.04 Unity 5.20. When I insert an audio CD, I get the dialog window asking me what I want to do. If I close or cancel that dialog window, then the CD icon disappears from the launcher. This only happens with audio CD's. Then it's clearly not a Unity bug. Try checking your autorun settings in the System Info thing in System Settings. Also, check if you have any other file manager installed that's fiddling around with this.
