Hi, I thought I contribute some additional info to this problem.

We have about ~50 nearly identical all-in-one PCs out there that are
experiencing relatively often the exact problem described here.

The industrial-grade hardware is based on Intel Atom N270 (1.6 GHz) -
see full specs here: http://www.cartft.com/catalog/il/1325

We use different kinds of non-mechanical hard disks:
- either a Kingston 8GB SSD SATA drive (most are using these, then we tried 
using other drives to avoid the problem we're talking about here)
- or a SATA-to-CompactFlash adapter with a 4GB SanDisk Extreme CompactFlash 
card (we use those cards - without adapter - in other configurations and never 
had any problems with them)
- or a Transcend TS8GHSD310 8GB SSD SATA drive

They are all running the same Linux image based on Debian 6, Kernel
2.6.38.6 and run 24/7. The system has been specifically modified to
avoid any unnecessary Flash writes. All partitions are r/w and
occasional writes are possible, but overall there are certainly far less
than 1 MByte writes per day.

Basically we're only using half of the 8 GB drive space so these drives
have a lot of room for wear leveling.

In the past months we had a number of failures that were mostly related
to the frozen SATA link described in this bug. The drives would not even
recover with a software reboot - the PCs need to be powered down and up
again.

We're using high quality cables and also tried to use different cables.
Note that these machines are firmly mounted, have no moving parts
whatsoever and are located in lonely places - i.e. there is no
mechanical reason why the SATA cable should suddenly have problems.

We only started recently to log in detail which hardware failed but I
can defenitely say that it happens relatively often with the Kingston
drives and it happened at least once with the CF adapter. I have no bad
records for the Transcend drives so far but only five machines are using
them so far. I wouldn't be surprised if even those fail.


My personal opinion is that there is some sort of incompatibility between the 
Linux drivers and the SATA controller or SATA drives that reveals itself only 
in certain rare situations. But other that that I'm clueless.

These machines most of the time do nothing but wait, so CPU time and
disk accesses are very low.

BTW, the same Linux image is used in a number of other machines without
SATA drives (they have CF card slots) and never had any comparable
problems. Also I was never able to reproduce the problem in our office,
which makes testing very hard.

and versions that work / not work fit my issue.
David: I wonder if you might post your logs. And does the issue still occur for 
you?

In my case, I have isolated the issue to revision numbers 2760 and 2761
of udev (launchpad reference), which was late in the 12.04 pre-release
cycle. I have been searching for someone else with the same issue.

after having changed from a legacy hdd to a ssd (ocz agility 3), running
Ubuntu 11.10, all cables are fine, but everytime there are excessive

The support from ocz told me, that I might have a chance resolving this by 
either applying a firmware update or the SATA3 to SATA2 fix. The latter 
seemed very interesting to me, could it be the main reason for the whole issue? 
I haven't tried yet since the ssd sits in an all in one pc that can't boot from 
usb, has a broken dvd drive, and is not really built for being repaired often, 
so I'd like to make sure it really solves the issue. And since I'm reading a 
lot about people having no problems with another OS I keep on waiting for a bit 
more ;) 
Anyone got a opinion about this sata3 to sata2 fix or whether it's OS or 
hardware connected?

I am seeing similar symptoms.
With a Sony Y Series laptop (containing an AMD/ATI SBx00 SATA interface), 
whenever AC power is removed I see similar messages in dmesg.

There are no issues at all under AC power, only when operating on
battery. The issue is present whether booting under battery power or
booting under AC and then switching to battery.

I suspect it may be a pm-utils or kernel regression (based on similar
bugs reported circa 2010).

The issue was not present with Ubuntu or XUbuntu 11.10 however is present in 
both Ubuntu and XUbuntu 12.04.
I have tried using 12.04 beta 2 and daily (as of 16/04/2012).
I have also tried using the most recent mainline kernel and still experienced 
the issue.

I will attach detailed logs once I have access to the machine again.

I went in the next computer store and came back with a new SATA data and
a new SATA energy cable(arround 20€ - propably cheaper when you have
time to look for).

I changed both against my old ones.

I have had runned the system since many days to be sure under haesy
hardrive load(virtulisation with qemu-kvm with many guests in parallel).

- No ata1 errors anymore with the same harddrive!

A deeper look at the cables shows me that the plastic connector of the SATA 
energy supply to the harddrive was deformed a bit.
It was nearly invisible but seems to be enough to interrupt the harddrive and 
causes this errors.



I would like to bring this issue up as a blueprint for Ubuntu/apport,
because it was a time stealing one which is not easy to identify.

The system should inform the user/superuser when there are entries of
this type in the kernel log that this affected with the possibility of
defect cable or cable connectors to the harddrive.

I let this open for now, so that user affected by this can find it.

With linux-generic 3.0.0.16.x Ubuntu serie is this problem not
observable to me.

Can not gather the Ubuntu12.04 guest logs.  The I/O errors have prevent
the logging/writing to the virtual harddrive.

Would it be possible for you to test the latest upstream kernel?  Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
v3.3 kernel[1] (Not a kernel in the daily directory).  Once you've
tested the upstream kernel, please remove the 'needs-upstream-testing'
tag(Only that one tag, please leave the other tags). This can be done by
clicking on the yellow pencil icon next to the tag located at the bottom
of the bug description and deleting the 'needs-upstream-testing' text.

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, 
please add the tag: 'kernel-unable-to-test-upstream'.  
Once testing of the upstream kernel is complete, please mark this bug as 
Confirmed.

@melchiaros regarding bug #958188, I'm not sure if mine is a duplicate
or not. Mine is specifically related to being an SSD, because I also
have a HDD installed which doesn't fail. Let me know if there's anything
I can do to help diagnose mine to see if it is at all related to this
(#965213).
