I have also experienced this problem. This seems to be a problem in Firefox 11.0's printing, as it did not occur in 3.6. This problem seems to arise only after at least one document has been printed already. This is most probably a problem of the application or its user interface library GTK, as you tell that it also happens when printing to PDF. The PDF printing feature in GTK's printing dialog does not involve CUPS at all. It generates a PDF and saves the PDF at a user-selected place. The url of the website is mailchip.com, but I am unable to supply my companies login details so you can access the report. I also get this sometimes when printing to the HP laser printer.
