If you want something long lasting, have it custom made.

Make the measurements before you call.

Try Gotham Sound in NYC. Speak to Alex in repairs. They'll ship anywhere.

OR ... just bite the bullet and get a Beachtek adaptor or something similar. 
It mounts to your tripod socket and has two XLR inputs with level controls 
and outputs to a single cable with 1/8 stereo plug for your camera.
