Since someone else mentioned GoodReads on this thread, I'll offer the
following: GoodReads is great for tracking books you've read, want to
read, or are reading. It's basically a social network for bookworms.
The app is completely accessible, or one could use the website. You
can create custom shelves, see what your friends are reading, and get
recommendations from friends or from GoodReads based on your ratings
when finishing a book. GoodReads is completely free and highly
recommended.

To keep track of the books I have read, I use an app called bookbuddy. It's 
perfectly accessible with VoiceOver and worth its price.

I track books I've read by putting them in a file.
I don't know of an app designed as a database that is user friendly.
