Have they tryed opening calendar. 
Then choose the event in question. 
Then go to edit. 
Then select the calendar button and then select family and then choose done?

The below scenario is under iOS 8.1 on both iPhone 4s and iPad Mini 2.

family member in question is also running iOS 8 and they have just the
4s.
They are all signed into their respective iCloud account where Family
Sharing is enabled.
I have a calendar event shared with members of my family, it is to take
medication daily from 21:00 to 22:00 with an alert 5 minutes before
and at
time of event. While I get the alerts, my family member doesn't. So
how do
they get them? As far as I'm concerned all settings are ok at my end. Is
there anything they need to do at their end?
Any help greatly appreciated. Thanks!

When calendar events are shared with others, they have to set their own
alerts for the event.  That's the way it works for us anyway.
