The dictation button will only show up if it's been turned on in Keyboard 
Settings.

I am preparing a presentation on mobile accessibility particularly with iOS and 
VoiceOver.

One of he features I expected was that when in safari and in a multi-line text 
area, that I would be able to dictate into the field, but when I double-tap on 
the field, the onscreen keyboard comes up but there is no dictation button.

Is this typical, or should there be a dictation button?

If you use the two finger double tap in a Safari text field you can dictate.
You are right about the button missing.
You should report that to Apple.  I usually do not dictate in Safari, so 
haven't noticed that before.
