Yes, I have noticed this.
I either touch the status bar and do a one finger double tap to bring focus to
the first item, or I touch where it says the folder name and swipe right, or
try the four finger tap near the top of the screen.

This may seem like a basic question, but I'll ask anyway. I have a large number
of apps on my iPhone 12 Pro, 296 if I remember correctly, and the majority of
them are placed in various folders. Recently, at least since IOS 15 was
released, I've noticed that when I open a folder, VoiceOver focus is
arbitrarily placed somewhere on the second or third row of apps in that folder
and VO will announce the item that has focus. `If I want the first item in the
top row of apps, I then have to find the heading for that folder and then flick
right to get to the first item, which is okay because I'd probably do that
anyway. My issue is with VO putting focus on an item that I have no intention
of launching. Has anyone else noticed this, and, if so, what did you do about
it?
