Yes, This seems to happen to me most often after completing a phone call as 
well. Weird" Turning VO off and on again does fix it, though.

I noticed a while ago that updating your apps can be pretty processor 
intensive on my older device. I went into settings and turned off 
automatic updates, so I can have my updates happening when I know my 
phone will be somewhat idle. I still have this problem occasionally, but 
it's much less frequent on my iPhone then it used to be. BTW, I hardly 
ever power my phone off and back on again. I do this after installing an 
update and the occasional time I have a problem, such as VoiceOver 
getting choppy, but I'd be surprised if I power off my iPhone more than 
once a week.
