Hello:

As per my experience, ringtones that I've purchased stay in iTunes but I 
must manually sync them to a new phone.  I would know as I've had to 
replace my iPhone 5S three times now.  LOL.

If you have custom ringtones on your 4S and they were not purchased from
iTunes you must have used iTunes at one point since that is the only way to
get ringtones onto an iPhone. You would have to have these ringtones in your
iTunes library on the computer or at least have them somewhere so you can
add them again to iTunes and then sync then to your phone.

If you have music on your existing phone you could sign up for iTunes Match,
this would upload all your music to your iCloud account (it does not count
against your 5 Gb free storage) and then you can in turn download the music
again to your new phone. If you don't want to do that and if you don't have
the music on your computer so you can sync it again, the only way to get it
off your 4S is with a program like Copy Trans which incidently costs just as
much as a one-year iTunes Match subscription and is not the most accessible.

If you Google something like Redownloading ringtones purchased from iTunes
you will find forum discussions which seem to indicate that for some reason
and unlike apps, ringtones cannot be downloaded again. I would, however,
write an email to iTunes 
Store support and simply explain that you are upgrading your phone, that you
purchased ringtones and that you are not using iTunes because you have an
older computer and screenreader. Ask them if there is a way to get your
ringtones back. The email address is as follows:
