Ron, A Sonicwall or Cisco ASA 5505 should fit the bill from a price/performance perspective. Please remember a couple of things:
1.) Firewalls typically protect your client at the "network layer" hence if your doctor's office has any interactive internet facing hosts (i.e.-web servers/web pages that accept user input) then a firewall isn't necessarily your best protection as most layer 7 traffic get passed through. (It isn't likely a that a small medical practice has interactive web apps but this is something you must consider if they do) 2.) The internet isn't the only vector by which your client can be attacked. For example, if they are running an unprotected wireless LAN, then the easiest path to the network could be connecting to their local AP. When installing a firewall you need to consider all the methods of entry into the network to determine where to position the firewall you want the firewall to protect.
