I really like the folks from Netsapiens.  We have a per line program worked out 
with them.  I pay for the lines as we use them.  Later, when we have enough to 
justify it I can just roll all of the lines into our own server.

For now, we pay a little more per line, but I have no server to worry about and 
nearly no startup costs.  Pretty slick.
