You can pick up a Sonicwall Firewall/VPN solution starting at about $150
from liquidators or $300-$400 from retailers.
  
The Sonicwall units are pretty easy to set up and they rarely crashed or
locked up (unlike cheap Netgear/Linksys solutions) ... It's been a while
since I've used one, but if I recall correctly, Sonicwall used a proprietary
VPN software which required users to buy a VPN client user license (about
50-60 bucks).

True on the license.  So I setup an MT133 as a VPN server on the inside, 
did 1-1NAT to it on the Sonic, opened the ports on the SonicWall and 
used the XP client.

I second the Sonicwall option.  I know quite a few schools, libraries, and a
few medical offices that use them.  Fairly easy to setup, very stable, and a
great value for what you get.  Go with the Enhanced OS though if you want
any sort of flexibility from the outside in.  

I know a few people that use Watchguard as well, but I'm not quite as
familiar with them.  My personal favorite is Checkpoint but their products
are higher priced than both Sonicwall and Watchguard.

Ron,

A Sonicwall or Cisco ASA 5505 should fit the bill from a price/performance
perspective. Please remember a couple of things:

1.) Firewalls typically protect your client at the network layer hence if
your doctor's office has any interactive internet facing hosts (i.e.-web
servers/web pages that accept user input) then a firewall isn't necessarily
your best protection as most layer 7 traffic get passed through. (It isn't
likely a that a small medical practice has interactive web apps but this is
something you must consider if they do)

2.) The internet isn't the only vector by which your client can be attacked.
For example, if they are running an unprotected wireless LAN, then the
easiest path to the network could be connecting to their local AP. When
installing a firewall you need to consider all the methods of entry into the
network to determine where to position the firewall you want the firewall to
protect.

Yes, you are right David, it was not specific. 

They need to protect their Medicial Billing Records, Patient info as well as 
critical info about their own business from Hackers who might discover thier 
business, damage some of the billing and medical data, or cause a failure in 
their system. Worst case would be to publish patient medical Records data, 
this has happened before and HHS and the Attorneys freak out, and so therefore 
do the Docs.

Outside Access requirement is only for the Doc's wife to access the Billing 
System (SW) to enable work from home.

I appreciate anything you are willing to share. And your pointing out the 
vagueness of the request was insightful, thanks very much.

That's incredibly vague. What do they need to protect, from whom, and what if 
any outside access should be permitted? This could be as simple as a $50 
Linksys router, or as complicated as a mid-range Cisco PIX (last I looked those

Sonicwalls are pretty hard to beat nowadays. The 170 series, 25 user models, 
can be gotten wholesale around $650 new.
A lot of our consultants are very happy with Fortigate, also.

The best choice is really a question of support, and your goal of what to 
deliver. You give them a Sonicwall, and your recommendation is backed by 
every other IT guy on the planet, that probably has experieince with it 
also.  Linux firewalls, are a bit less intuitive for people not specializing 
in Linux.  If you are intending to manage their network, MT's can be a good 
way to offer value, powerful features, and remain more valuable yourself in 
the future maintenance operations.
