It sounds like it's not a Xinerama specific problem then.

I'm more interested in the non-Xinerama case because it's
less involved. You're saying this app worked with a three headed
non-Xinerama server prior to XFree86 4.2, but breaks with
4.2? This is the same version of the app in both cases and
not a newer one?
Do you have a debugger backtrace from the app? It would
be nice to know where it died to maybe get an idea what
kind of stuff it was doing.
