Yes, that was the issue.
In the V13 version of the code, SET MENU BAR(1;*) was tucked away in the
bowels of the On load code. This version of the palette is vastly
simplified and, in my haste, I deleted that code.

sounds like you are running the Request() function in a process for which you
didn't install a menu bar (SET MENU BAR)

Yes. The Standard Action is paste, Ctrl-v is the shortcut, and the item is
enabled.

In the V13 app, the menu bar switch from the Design menu bar to a menu bar
that has only File, Edit, and Help menus. In V15, there is no active menu
bar. Hmm…

exact details escape me.

to update a utility palette that I've been using for some years. One of the
functions opens a request dialog into which I paste a string
