my railway consession was issued in 2011 and it is still valid.
i did not get consession on Rajdani express i.e. 25 percent due to
abscent of the doctor's name and registeration number.
this new norm was recently updated by the railways of the doctors name
and registeration.

The old form can still be used until it expires, However absents of
doctor's name is an appropriate reason for rejection. So I am afraid
that you have to get a new form.

I hope you all are aware that on 14 June 2014, Railway Board issued a
circular integrating all concession forms for the disabled in to one new
format. All existing concessions certificate not carrying the essentials
like Doctors seal, name and registration numbers etc would not be valid.
For ease
please correct me if I misunderstood. I opened the given link ,it
states concessional certi issued before 15th july 2014 ,would be
considered valid. secondly ,in the concession format ,on the 2nd page
,there is a # marked instruction that states for blind persons
