and I have not changed any management classes. My servers being backed
up are mature so they reached the point the generally when
extra versions - 90
retain only version - 90
(for the 9 day retention, change 90 to 9).
Backups of databases and incrementals of everything else are run
nightly.

You might want to check your Maximum Size Threshold on your disk storage
pools. If files are larger than the max, they will go directly to your
primary sequential pool and bypass the disk pool (and therefore not get
to the copy tape pool until the tape to tape runs).

I have some application commands that must be run before backing up a
client (w2k server) and it's DB. I run this out off a .bat script that
I execute through the tsm backup schedule. The normal commands to
shutdown the db, the dsmc backup command and the db startup work just
fine in the script.
Try to do a checkout. I suspect it will not be found either, meaning
your tsm DB is not in sync with your library. Manually take the tape
out of the library and then check it in. You may also have to do an
audit on it once you check it in.
