Thanks for getting back to us with the requested information.
Upon checking the attached logs, it seems that the Add Lead Form Extension
feature is available to allowlisted accounts only. Also, note that Lead forms
require that the Terms of Service have been agreed to before mutates

libraries > Your client library (ex. Java) > Logging
