You pay for the total length of their fiber that you're using.
In my case I actually did want to get from Town A to B to C, and they
counted each leg as a separate IRU. The points where we stop at a
splice case were the ends of their run. So A-B was one billing item and
B-C was another.
I think a yearly maintenance fee is the typical arrangement but have also
heard of a strand leesee or maybe IRU owner needing to share a portion of
the costs for, say, a cable relocation due to road widening. All depends on
the agreement in place.

You don't get to access their cases, so you leave them a pigtail and
they splice it into their system. Often you pay each time you need them
to make a connection for you (hopefully just the once).

I'm not in the dark fiber game, but what do you mean by 'per-event'
billing? Like if there's a fiber cut, you're responsible for part of
the repair cost? Or is that for like accessing the electronics on the
ends of the fiber.

That’s not bad, on par with some long term dark fiber rental contracts I have
currently.
