I've opened a ticket with BMC on this one. I will be adding this finding
shortly, but I thought I'd share and find out if anyone else ran into this.

We loaded java jdk 8u65 in "D:\Program
Did you set the Java_home variable ? In Windows env?

Hi Anonymous,
This might be a browser cache issue - open the form in Mid-Tier and press
Ctrl+F5 to force the browser to get a fresh copy of the form from the server
instead of using it's cache on your PC to display the form.
