I had exactly the same question. There is some really useful documentation
on voip-info.org regarding the extensions.conf syntax.

call has been hungup.
I've had a look on voip-info and checked the conf file
EEK! Doesn't handle SIP correctly, did they explain what they meant?

I had the same problem. It turned out that X was hogging all the resources
available. Running without it, the problem cleared up.
