I have been experiencing a similar problem.
I have not yet been able to figure out
what the exact problem is but I know that the errors are inconsitant.
Sometimes nothing for 2 days and sometimes
5 times a day.
I thought about it a lot and I have found
only one thing in common.
The

of a billing software.
I have made a card for each of my
customers and the call through and get authenticated with callerID and then the
call is placed using dnid.
This is working great.
The problem that I just ralized is that
when I get a call

its ok and I found them to be by far the cheapest provider in Australia.
They offer DIDs at $1.95 per month.

Just go into asterisk cli and type show g729
It will tell you how many are active and how many you have in total
