Probably a layer 1 or 2 issue.
Look at the stats for the port on the Cisco switch, there may be clues
there.
Check the speed and negotiation settings for the port and NIC, lock it
to 100MB.
Change the patch cables (DO NOT use hand-made cables)
Change the NIC in the Asterisk server
