You are going to have to roll up your sleeves and dig deeper yourself.
It sounds like /etc/asterisk/extensions.conf is correct but Asterisk is not
using it. I'm not sure how that could happen.
Take a look at your /etc/asterisk/asterisk.conf and make sure nothing is funky
there.
