I'll give it a shot, since no one has responded. Check your user account control settings in control panel. Is it, by any chance, on 34%? If UAC is at 34%, screen readers won't be able to interact with it, as while the UAC pops up on the screen

UpdateI just figured it's not something with nvda, but with the narator.If I'll press alt + a before the narator starts speaking, everything works fine, but when it starts, no way!

Does, say, narrator work on the secure screen? I kind of have an idea of what it is, but want to make sure it's not from the screen reader, otherwise it won't be relevant.

No addons, and on nvda.ini is just one line, schemaVersion = 2, if you mean the program files x86 one.
