It works fine with NVDA, where fine means you'll have to fiddle with it a few times. The trick is that you want something like file->scan, then to immediately export your document, as opposed to trying to use their editor th

Not a lawyer, but the app you want is Abbyyfine Reader. it's a bit finicky with accessibility and not the cheapest thing out there, but if you get a good feed scanner it beats all the blindness-specific apps like Kerzweil
