@3, Not necessarily. If you have Virtual Audio Cable, you can set it as your default device, set it as your desktop audio device in OBS if that's what you're using to record the game, then set NVDA's speech to play through the headset

you will need a mixer for this, and you can set the sound to go true all sound devices connected to the mixer.

You can select NVDA's output device by going into the synth dialogue. Press tab and you'll be taken to the list. For system sounds, type mmsys.cpl into run and press enter. Press select default next to the device you'd like to use.
