The title of the book explains a lot. "Registration Book MUNICIPALITY of
Ponta Delgada, Azores, Portugal." That would have to be Sao Miguel island.
The other Ponta Delgada on Flores island is not a "municipality" (concelho)
but a freguesia (village) in the concelho of Santa Cruz. It

The Register of Birth is the translated document from the department of Ethnic
Affairs Commission in Australia. The registration is Paragraph 141 of
registration book Municipality of Porta Delgada Azores Portugal.
