Row 19, in the column titled "The NAME and complete address of nearest relative from whence alien came," it says Francisco F. Lawrence, Rib.a Tainhas, V. Franca. The column isn't asking for parents, it only ask for A NAME. Just one name. If you want mom's name, you have to go to the Azorean Passapo
