To add to this, apparently this tape is just bad or needs to be erased. I
marked it read only and mounted another tape into drive 0 and everything seems
to be working. Is there any way for bacula to just say, "Hey this tape is
crap, send me an email and load another tape"

I have been using bacula for some time with an overland neo 2000.
However, Sometimes I receive these errors:

libraries as well and you can get them with LTO3 and LTO4 drives.
I think pretty much anything that supports mtx or mt commands is compatible
with Bacula, of course anyone more knowledgeable than me may know more about
