If I change single_dir_install to 0 or just leave it out, the build completes
and ends up with the proper RPMs. Does anyone
know what's happening, or better, how to stop this from happening?

OK, I just downloaded qt-devel and tried again. Still bombs in the same places

If downloaded means that you installed the rpm, then that is surprising.
Does /usr/include/QtGui/QAction exist now?
Which version of Qt is this?

I downloaded/installed
