I added a third commit to my fork:

it is not showing in the pull request. Can someone help me get this commit
added to my original pull or do I need to create a new pull request ?

Have you tried the base case? I.e. calling logout directly from your
browser taking your application's configuration out of this?
This issue seems to be isolated to you as of right now (we haven't had any
reports at least), so I recommend going back to the basics to confirm it
works and then slowl
