My gues would be the pstn pri does not have 12 channels configured.
The degustation is showing that the t1 is taking channel 12 and it is being
rejected by the router.
To test this theory change the bchannel order around and I bet it works...

Phones are getting the ip address and option 150 from SC router DHCP.
However the phones do not show DN or extension. (only blue image background)
When I checked the Network setting in phone (ip,subnet, TFTP all showing
correctly)

I think the above config on hq-rtr will prevent it from
using the below
pri-group timeslots 1-12,24 service mgcp
hence not using only first 12 channels,
try to remove the ccm-manage config and config server.

Where are you getting your dhcp at sc? If it is sc rtr then you could have it
right and just no access to CUCM
So try ping (CUCM ip) source vlan (voice #) and confirm the voice vlan has
connectivity to CUCM

is it registering and not getting a dn?
This seems sort of strange so I might look at replication and where you are
registering if the phones register.

I experienced the same thing when studying for the IE in my home lab. The
solution for me to get rid of the very blue screen was to check the background
image and revert to the standard image. Then my phones showed showed correct
information.

I'm curious, does anyone has feedback on this titles. I have always been
an avvid reader and I like to see a different angle from time to time.
I have search on amazon, for CCIE related non-tech titles, to get into the
mindset, and do some visualization but most have poor reviews.

Try upgrading or downgrading the phone firmware, then restart the tftp
service.
If it still doesn't work, try to register this phone on another call
manager.. If it still doesn't register, it might be because the phone is
messed up inside..
