Your debug output has a few clues...but I can't recall offhand if channel 16 in that debug starts at 1 (meaning this is the 16th channel) or 0 (meaning this is the 17th channel). Do inbound calls from pstn work? If yes, its more likely the second option. In the first case, it would appear your
