I actually saw a problem like this a year or so ago.
This may sound silly, but try moving your hidden field down to the bottom
of the form just before the submit button. I know this sounds ridiculous
but I actually had this problem and this is what fixed it. Please let me
know if this works

The receiving template is pretty much irrelavent because all Im trying to do
for debug is test the presence of the tw
