Am trying to run urpmi --auto-select and it is saying "everything is already
installed" even
though I have checked and there are newer versions of installed packages available.
Went into software sources manager and did a manual update but still nothing. Tried
urpmi.update but this doesn't work (i reported it in a different bug report).

There appears to be nothing wrong according to medium register, maybe there has
been a problem of upgrading package, can you tell me which package was newer ?

There were lots of newer packages, amongst them stuff like drakxtools,
drakxtools-newt, kdebase etc. (It was a few days since my last urpmi --auto-
select).

Well, just now urpmi --auto-select ignored kdeutils-3.1-5mdk.i586.rpm
libalsa2-0.9.0-0.13rc7mdk.i586.rpm procps-3.1.6-1mdk.i586.rpm which
where updates that rpmdrake did find and install, without any
intermediate urpmi.update.

This is a problem of hdlist not synced with rpm I suppose because urpmi cannot
do better than what is available in synthesis file, ie it is during a sync of
mirror.
As the version in rpmdb is exactly the same as the version in synthesis files
for package you know there was better.
