log error message when the reach provider fails (#205)
* log error message when the reach provider fails
- clean up require statements
* Set response status to 500 on reach doc error
- log error in the main http post function instead of the fill doc function
* rethrow reach erros after logging them
* log timeouts for reach service
- logs error when reach takes too long to respond
- adds reach response time limit to config
- todo still need to change server response to 504
* send 504 when REACH times out
* Log when reach gives back no recognized entities
* General error logging for reach
- remove timeout specific reach error logging
- log successful doc creation
- log error messages on separate log error lines
