Looks like pivot_root can't find the root filesystem - a common problem
but the error is unfortunately unhelpful.
I had this for a USB HD because the proper usb modules are not added to
the initrd created on the installed disk.

hd-media is different then the 'default' netinst image.

I'm no expert but this sounds similar to the problem I had booting off a
USB hard drive that I had installed to - only difference seems to be you
require a custom module whereas I just needed to pick up some which were
there but were not being used. Have a look at the instructions in
Package: installation-reports
