AFAICS, Debian mkinitrd does not automatically load the usbhid module, needed to access the keyboard, and I don't see usbhid or the prerequisite controller listed in the script listed earlier in this report. To work around this, add usbhid and ehci, ohci or uhci to /etc/mkinitrd/modules; of course the more general kernel problem may also exist. If you're interested in adding USB kbd support to mkinitrd, I'm working on a perl based alternative mkinitrd implementation that handles USB keyboard.
