I've been struggling with coldplugging and the new udev as well, but this 
report baffles me. There is nothing in libsane.rules that should modify 
anything in /proc/bus/usb, since it runs no scripts, but just sets the 
permissions of /dev nodes (unless I've totally misunderstood what udev 
rules do). Thus I don't understand why hotplugging a scanner only after 
boot alters /proc/bus/usb (as you report; I have no USB scanner to test).

This might be a bug to refer to the sane-backends maintainer, who can 
presumably verify if coldplugging is working with the new udev, and can 
clarify the importance of /proc/bus/usb, in addition to the /dev nodes.
