I have now looked carefully through mktexlsr and traced its execution
to try to get to the bottom of this.
All I found was that various ls-R* files were wrong. Removing them
allowed mktexlsr to run properly and all the texlive updates to
complete.
I have no idea how the incorrect ls-R files
