In etch I upgraded the libc6 last night:
[UPGRADE] libc6 2.3.6.ds1-13 - 2.3.6.ds1-13etch2
Since then pam_mysql doesn't work at all. Similar log as earlier.
authdaemond: PAM unable to dlopen(/lib/security/pam_mysql.so)

I was trying to use libpam-mysql for authentication which never worked
for me. The following message appears in the log when trying to auth via
pam. Authentication only worked with other methods (pam_unix).

A solution hack is also given:

I encountered the same problem today with postfix using SASL for SMTPD after
upgrading.
Meaning I couldn't send my email with any desktop client through that server
anymore.
Log messages were at first the same as Simon's (from postfix): unable to open
Berkeley db /etc/sasldb2
